NALZS Liability Waiver
LIABILITY WAIVER AND RELEASE FORM (MINOR CHILD)

I hereby certify that I am the adult parent or guardian of ( type minor child name below), a minor child under the age of eighteen years, and I consent to his/her participation in all camp activities at Little Tykes Day Camp (the “Camp”) located at the 603 S Jefferson St, Athens, AL 35611. The camp operated and maintained by the North Alabama Zoological Society, (“NALZS”) and the property is owned by a third party.

I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury, property loss or damage) of (1) said minor child’s participation in recreational activities at the Camp. I recognize my responsibility to ensure that said minor child participates only in those activities for which he/she has the required skills, qualifications, training, and physical conditioning. I understand that NALZS, Athens Limestone Library, city of Athens, the property owner shall have no responsibility to pay for medical treatment and related costs if said minor child is injured.

Knowing the risks described above, I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s attendance to the camp. To the fullest extent allowed by law, I hold harmless and agree to indemnify NALZS, its officers, directors, faculty, staff, volunteers, employees and agents, from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor child’s participation in camp activities at the Camp, resulting from any cause whatsoever, and regardless of fault.

I understand and agree that no oral or written representations can or will alter the contents of this document. I agree that this agreement shall be governed by the laws of the State of Alabama.
NALZS Photograph Waiver:
PHOTOGRAPH AND IMAGE WAIVER AND RELEASE FORM (MINOR CHILD)

I, hereby grant and authorize North Alabama Zoological Society the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of my child to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats and markets now known or later discovered. This authorization shall continue indefinitely, unless I otherwise revoke this authorization in writing. I waive the right to inspect or approve any finished product in which their likeness appears, including written or electronic copy. I waive any right to royalties or other compensation arising or related to the use of the photograph.
